CONSENT FORM:
By submitting this form, I authorize my child to participate in the Little Bucs Cheer Camp and acknowledge that I have read, understand and agree to all the information stated in the consent form.	CONSENT FORM: I hereby state that my child is physically fit and has my permission to participate in all camp activities. I also understand that even though the chances of an injury are reduced to a minimum, the possibility of injury is still there. Therefore, by signing this waiver form, I release Cobb County School District, Allatoona High School., Coaches & students from any and all legal action in case of an injury to my child. In addition, I hereby give my permission to seek emergency medical treatment in the event I can not be reached. I also further understand that I the parent/guardian am responsible for all medical bills incurred as a result of participating in all cheer camp activities at Allatoona High School.
